The PALS Diabetic Program

Diabetes Management is a critical component in the overall care of many older adults. The PALS™ Diabetic Program offers a complete care program designed to enhance the health and well-being of residents in Assisted and Independent Living Communities with Type 1 and Type 2 diabetes.

 

Our comprehensive program provides:

  • Geriatric-, Home Health- Diabetic-trained nurses
  • A Medical Director, Dr. John H. Fullerton (Geriatrician/Internist) certified by The American Medical Directors Association (AMDA) and American Gertiatric Society (AGSF)
  • A Clinical Director trained as a Diabetic Educator (Kate McElroy Fullerton, PA-C)
  • A Program Clinical Director, Kate McElroy Fullerton, PA-C and her associate,  Alexis Nicole Guy, PA-C trained as Certified Harvard University Healthy Life Style Coaches with an emphasis on food choice and integration of exercise
  • Patient/Family and staff education on diabetic patient care in the Assisted Living and Independent Living setting, with emphasis on Hypo/Hyperglycemic reactions and emergency responses
  • Our clinical and medical team working in conjuncton with the resident's current Primary Care Provider and/or Endocrinologist and reporting any 'change of condition'
  • Materials and resources certified by AMDA which includes a diabetic tool kit with policies and procedures for Assisted Living
  • Nursing services provided 7 days a week
