Theresa Kemnitz, D.O.
Associate Medical Director
Dr. Theresa Kemnitz completed her Internal Medicine Residency at St. Mary's Medical Center in San Francisco as well as her Assisted Living Geriatric curricula under the direction and mentorship of Dr. John H. Fullerton, Education Director of Geriatric Medicine for St. Mary's and Founder of Hampton Health, Ltd.™
Dr. Kemnitz is the Associate Medical Director of Hampton Health, Ltd.™ and on its behalf serves as the Medical Director to Marin Convalescent in Tiburon, CA and Nazareth Vista Convalescent Hospital in Belmont, CA.
In 2010, Dr. Kemnitz was chosen by Dr. John Fullerton (along with her particular emphasis on preventive medicine and humanitarian care) as the On-Site Visiting Physician to Atria Lafayette (formerly The Stratford at Countrywood) in Lafayette, CA where she served until 2016 and where she was a member of the Medical Advisory Council for Hospice of the East Bay located in Pleasant Hill, CA.
Dr. Kemnitz provides primary care and long term care medicine to some of the finest skilled nursing and rehabilitation facilities in Marin, including Professional Post-Acute in San Rafael, The Redwoods in Mill Valley and Novato Healthcare Center.
She additionally provides on-site, geriatric-focused visiting physician services to the Alzheimer’s and dementia care communities Windchime of Marin in Kentfield and Harmony House in San Rafael, and to the Assisted Living communities Vintage Coventry and Vintage Golden Gate in San Francisco.
Dr. Kemnitz graduated from Touro University Medical School with a sub-specialty in the musculoskeletal system designed to address the needs of preventative and holistic medicine. Having provided long term care nursing at Abbott Northwestern Hospital in Minneapolis, MN she previously received her RN degree from St. Catherine's University in St. Paul.
Her expertise in the field of long term care began with a degree in Physical Education and Sports Medicine from The University of St. Thomas in St. Paul, working as an After-Care Coordinator helping bridge physical therapy to mainstream maintenance programs.
Dr. Kemnitz had always wanted to provide daily, on-going care to people and her local community, but it was her work with specialists at Abbott Northwestern Hospital that inspired her to become a physician, which has given her the opportunity to provide care to her patients and community on an even deeper level.
Her additional duties at Hampton Health include serving as a preceptor to St. Mary's Medical Center Internal Medicine Residents and Samuel Merritt University Physician Assistant students (PA-S) from the Primary Care Associate program, each rotating in Geriatrics through the practice.
In March 2013, Dr. Kemnitz joined the Hampton Health, Ltd.™ Clinical Research Division serving as a Sub-Investigator for the Avanir Pharmaceuticals 'Prism II' Clinical Trials Study. She holds a Certificate in Advanced Wound Debridement and has been a primary care physician with Hampton Health since 2009.
Long Term Care
Dr. Craig P. Giacomini joined Hampton Health, Ltd.™ in January 2015. Dr. Giacomini completed his B.S. in Biology at Stanford University, and his MD and PHD (Cancer Biology) at Stanford University Medical School.
Dr. Giacomini trained in Internal Medicine at St. Mary's Medical Center in San Francisco and subsequently began a Residency at UCSF in Radiology. While completing his Internship at St. Mary’s he trained and completed his Geriatric curriculum under the direction and mentorship of Dr. John H. Fullerton.
Also during his Internship at St. Mary’s, Dr. Giacomini received awards for ‘Intern of the Year for Inpatient and Outpatient Clinical Medicine’ in addition to his history of numerous Medical Scholars Research Fellowships, the HHMI (Howard Hughes Medical Institute) Medical Fellows Program Fellowship and academic honors throughout his time at Stanford.
For Hampton Health, Ltd.™ Dr. Giacomini has provided medical care to patients within our private practice in the San Francisco and Marin County locations as well as in the South Bay at the Skilled Nursing Facility Nazareth Vista Convalescent and its sister Assisted Living Community on the same property in Belmont, CA.
In addition, Dr. Giacomini has functioned as a preceptor to St. Mary's Medical Center Internal Medicine Residents and Physician Assistant students (PA-S) from the Samuel Merritt University Primary Care Associate Program, each rotating through Hampton Health in Geriatrics.
Jenna Keohane, MSN, AGPCNP-BC
Adult-Gerontology Primary Care Nurse Practitioner
Jenna Rose Keohane, MSN, AGPCNP-BC is an Adult-Geriatric Primary Care Nurse Practitioner Board Certified through the ANCC (American Nurses Credentialing Center). She completed her Master of Science in Nursing and Nurse Practitioner Specialty studies at Vanderbilt University School of Nursing in Nashville, TN and received her Bachelor of Science in Biological Sciences at the University of California, Santa Barbara.
Jenna was inducted into the Sigma Theta Tau International Honor Society of Nursing, Iota Chapter, in April 2016 and is an Active Member of the American Nurses Association, the American Association of Nurse Practitioners and the California Association for Nurse Practitioners.
Her professional positions and volunteer experience have included Peninsula Gastroenterology in Redwood City, CA and Dr. Gerard Butler Pediatrics and Adventures in Caring, both located in Santa Barbara, CA.
Before completing her Master of Science in Nursing and Nurse Practitioner Specialty studies in Tennessee, Jenna trained as a Nurse Practitioner at the Neighborhood Health Downtown Homeless Clinic in Nashville, working with the uninsured and underserved, seeing patients with a variety of mental illnesses and managing severely uncontrolled chronic diseases.
Jenna also assisted Albert Brandon, DO at the Manchester Rural Clinic in Manchester, TN in connection with her studies, where she independently performed complete and focused physical examinations and histories and managed patients with multiple chronic diseases in a rural primary care setting.
Jenna Rose Keohane, MSN, AGPCNP-BC was welcomed into the Hampton Health, Ltd.™ primary care clinical team in January 2017.
Hazel Tagad, RN
Care Manager - Hampton at HomeSM
Hazel Tagad is a Registered Nurse who spent numerous years at Aegis of Corte Madera as the Assistant Wellness Director, where she worked side-by-side with the Hampton Health, Ltd.™ on-site medical practice and clinical team.
After years providing care as an LVN at Aegis and with Sutter Visiting Nurses Association (Sutter VNA), providing home and community nursing services within Marin County, Hazel left her position to complete her degree as a Registered Nurse.
Currently, Hazel serves as a Care Manager with the Hampton Health, Ltd.™ division Hampton at HomeSM, providing nursing care management to private home patients.

Glenn Perseveranda served Hampton Health, Ltd.™ as a Medical Assistant and Certified Phlebotomist from January 2015 until October 2016 during which time he completed his Licensed Vocational Nursing degree.
In October 2016, Glenn began work as an LVN at Novato HealthCare and continues his relationship with Hampton Health as a Care Manager through the Hampton at HomeSM division.
Prior to joining Hampton Health, Glenn had a long work history as a Resident Caregiver and Certified Nursing Assistant with communities and skilled nursing facilities among them Summerville Senior Living in Fairfield, CA and San Rafael Convalescent Hospital in Marin County.
